Holly Madison Gets Candid About Her Lower Facelift at 46


If there’s one thing Holly Madison has always been, it’s candid. From her years on the early-2000s hit The Girls Next Door to life now, the former Playmate has never shied away from talking about, well, anything. In a new interview with Tucson dermatologist Sheila Farhang, MD, on her podcast Derm Approved, she added a surprising entry to the list: a lower facelift.

“I’m really happy I got it done,” Madison told Dr. Farhang during the episode, which aired on Wednesday, June 24. She shared that she underwent the procedure on her jawline about a year ago, along with a little liposuction under her chin.

“I just felt like after having kids, you know, I gained a lot of weight with my second pregnancy, and there was just nothing I could do,” Madison said of the decision. “There were no lasers or anything that could really do what I wanted it to do.”

She said she did her research before choosing a surgeon, ultimately going with someone whose before-and-afters she loved. Her goal was to address jowling and tighten the lower face, but with a conservative approach. “I’d rather have it not go far enough, and then I can go back and maybe do more rather than have it go too far, and I’m looking like the Joker,” she explained.

The recovery was easier than expected. She took about two weeks off from filming her podcast and recovered at the Beverly Hills Hotel with a nurse. Madison noted that the scars sit in front of her ears, but she’s not trying to hide them. “If you look close, you can totally see it, but I don’t care,” she said. “I’m kind of open about it. I already talked about it on my podcast.”

That same less-is-more mindset seems to carry through much of her beauty routine. Madison told Dr. Farhang that Botox is still part of her regular maintenance, along with IPL laser, and that she recently tried CO2 laser under her eyes, which she said helped with the “static wrinkling” she was seeing there. She also admitted she’s less sold on microneedling, saying it’s one of those treatments she does more on “good faith ” than because she sees a dramatic difference.

And while Madison has been open about what’s worked, she didn’t hold back on what hasn’t. Her biggest regret? Hand rejuvenation. After trying the treatment, she said the initial results looked promising but didn’t last. “I did the whole laser and filler combo, and then your hand kind of swells up the first day and looks like a hospital glove with ice in it, and then it goes down, and I had the cutest chubby hands,” she explained.

“I was so excited, but that was just the swelling, and it went away. These hands are as bony as the day I walked in.”
